How do I file a claim?
Upon the death of the Insured, the policy beneficiary (or individual assisting the beneficiary) should notify us that the Insured has passed away. Please contact the Centrian Life Call Center toll free at 1-800-315-0552. A representative will be available to assist you with the claim process. Our representatives are available each business day, Monday - Friday from 8:00 A.M. - 6:00 P.M. (ET).
The caller should provide us with some general information, including the following:
- The insured's date of death.
- The address to which we should mail the required claim form(s).
- Any information that can be provided relative to the beneficiary(ies), (i.e., who they are and whether or not they are still living). The names of all beneficiaries will have to be verified by us.
After reviewing the policy records, we will mail the required claim form(s) and instructions for filing a claim to the beneficiary(ies) at the address you provide.
