How do I change my name on my policy?
Whether you're the Insured or the Policyowner, it is important to notify us of any change of name. If you have recently become married or under some other circumstance have changed your name, you must complete the required Name and Address Change Request Form. The change form must be completed as specified and signed by the policyowner prior to returning it to our office for processing. Please note: You will not be sent a new policy. Although your original policy bears the "old" name, this policy is still valid.
