How can I find out if my deceased relative had life insurance?
If you are not certain if your relative had life insurance, there are a few things you can do to try and find out if she/he had a life insurance policy:
- Contact other family members (i.e., brothers, sisters, or children) who may have known if your loved one had life insurance.
- As part of the probate process that you will most likely undertake after your loved one's death, you can check to see if a life insurance policy was listed as an asset.
- Go through canceled checks or contact your deceased relative's bank for copies of old checks. Also check credit card statements. You want to see if there is a record of a premium payment that you can trace back to Centrian Life.
- Contact any past employers to see if your relative had group life insurance.
- You can also check with MIB Group, Inc. The MIB maintains a database that may indicate that your loved one applied for life insurance within the past seven years.

How do I file a death claim?
Upon the death of the Insured, the beneficiary, or individual assisting the beneficiary, should notify Centrian Life that the Insured has passed away. Please contact the Centrian Life toll free Call Center at 1-800-315-0552. A customer service representative will be available to assist you with the claim process. Centrian Life representatives are available each business day, Monday – Friday, from 8:00 A.M. - 6:00 P.M. (ET).
The caller should provide us with some general information, including the following:
- The Insured's date of death.
- The address to which we should mail the required claim form(s).
- Any information that can be provided relative to the beneficiary(ies), (i.e., who they are and whether or not they are still living). The names of all beneficiaries will have to be verified by us.
After reviewing the policy records, we will mail the required claim form(s) and instructions for filing a claim to the beneficiary(ies) at the address(es) you provide.

How quickly does Centrian Life usually settle life insurance claims?
Once Centrian Life receives a completed death claim and a death certificate proving the death of the Insured, the beneficiary usually receives a death benefit within one week.
The policy is contestable for a two year period for suicide, fraud, misrepresentation and lack of good health.
